1) PTA Membership Form You can download it here, or pick up a form in the Cresthaven Elementary schools main office.

 

2) Fill out the PTA Membership Form.  Please print clearly. 

 

3) Staple the $10.00 cash payment to the membership form and either return it through your student to the teacher or place it in the white PTA box located in the school office. If you are a Grandparent or Community Member who would like to support the schools PTA you may also mail your money order to 801 NE 25th St. Pompano Beach, FL 33064. 

 

4) The PTA will return your new membership card through your students backpack within 2 weeks.
